Evaluation Process
During the evaluation our team of specialized therapists will use clinical observations, family interview, and standard assessment to determine if services are appropriate. Our evaluation takes approximately 30 minutes to an hour maximum. We ask that parents wait in the waiting room during the evaluation if the child is able to separate and attend alone. The last 15 minutes of the evaluation will be used to discuss observations and discuss recommended & next steps. Families will receive a detailed report within 2 week from the date of the evaluation.
How to Prepare
All paperwork must be completed prior to scheduling the evaluation. Please submit a photo of your insurance card and any other documentation requested such as previous evaluation and IEP plans.
Cancelling & Reschedules
If you need to cancel or reschedule your evaluation please contact the front desk at least 48 hours prior to date of evaluation. We will do our best when rescheduling your appointment to assure evaluation occurs as soon as possible. If your child or someone in the house is sick or experiencing COVID-19 symptoms please contact the front desk to reschedule appointment.
